To start using the application simlpy upload the included folders and files to a folder on your website.
Folders 'reports' and 'wizard'
File 'index.php' 

Navigate to the folder you uploaded the folders and file in your browser.
www.your_site.com/your_reports_folder/index.php

Depending on your web host you may need to chmod the folder reports to 777 or r/w/e

You can add any additional sql for the statistical function simply by adding it in the functions.txt file.
